The medical certificate in form 1A for DL renewal does not contain the doctor's phone number,doctor's location.Doctor's contact number is required in form 1A.I contacted the medical team and person named ms.Uma and there is no response.Without relevend details of the medical officer the mc is a waste and can be doubted to be fake
